Job Summary

Reporting to the Manager Transitional Housing the Case Manager supports women navigating the impacts of homelessness mental health challenges poverty addictions violence and abuse. This role delivers trauma-informed client-centered case management that empowers women to thrive through emotional support safety planning goal setting skills development referrals advocacy and systems navigation. The Case Manager works collaboratively with the team our clients and community partners to foster resilience and independence ensuring practices align with YW Calgarys mission vision values and strategic priorities. This position plays a vital role in creating a safe supportive environment where women can rebuild rediscover their strengths and achieve long-term stability.

This is a sixâmonth term opportunity working Sunday through Thursday 9:00 a.m. to 5:00 p.m.

WHAT YOULL DO

  • Build trusting professional relationships using trauma-informed principles that prioritize emotional safety choice collaboration and empowerment
  • Conduct comprehensive intake and ongoing assessments to identify client strengths needs risks and goals related to housing safety mental health substance use relationships and overall wellbeing
  • Develop implement and regularly update individualized case plans in collaboration with clients ensuring plans are goal-oriented trauma informed and reflective of clients evolving needs and capacities
  • Formally meet with each client a minimum of one to two times per week to monitor progress provide coaching and ensure alignment with case plan objectives. Adjust plans as needed to reflect changing circumstances progress or emerging risks
  • Facilitate regular safety and risk assessments developing individualized safety plans that prioritize physical emotional and cultural safety while empowering clients to take an active role in their own wellbeing
  • Deliver ongoing crisis intervention and supportive counselling using a trauma-informed strengths-based approach to de-escalate situations promote self-regulation and connect clients with appropriate supports
  • Provide support to clients during times of crisis assisting with immediate response stabilization and post-incident debriefing to promote learning emotional safety and recovery
  • Provide advocacy and systems navigation support to help clients access income supports healthcare mental health and addictions services legal resources education and employment opportunities
  • Coordinate case conferences and consultations with internal team members and external service providers to ensure wraparound collaborative care for complex client cases
  • Support clients in building practical life skills such as budgeting communication conflict resolution time management and household maintenance to foster independence and stability
  • Oversee client intakes transitions and exits ensuring all associated surveys assessments and documentation are completed accurately and within established timelines
  • Provide general oversight of service delivery to ensure alignment with social work best practices trauma-informed care principles and ethical standards of professional conduct
  • Oversee all referrals to external providers and practitioners ensuring coordination continuity of care and appropriate follow-up
  • Ensure excellent organization and management of all client case files maintaining up-to-date and well-structured documentation that supports continuity and quality of care
  • Support program operations by assisting with tenant-related administrative tasks such as rent collection rent tracking and follow up as required.
  • Manage front desk to cover breaks and as needed
  • Integrate equity diversity and inclusion principles into ones role work and engagement with others as aligned with YW Calgarys EDI strategy and values
  • Understand YW Calgarys Health Safety and Wellness (HS&W) policies practices and protocols; ensure work is performed safely and in alignment with YWs HS&W culture

QUALIFICATIONS

  • Bachelors degree in social work counselling or related field
  • Registration with relevant professional association is a requirement e.g. ACSW
  • At least 2 years of experience working with vulnerable women and populations experiencing complex mental health addictions family violence trauma and homelessness
  • Current CPR and First Aid Certification ASIST NVCI training is required upon commencement of employment

  • Demonstrated experience assessing complex clients helping identify various needs and strengths and creating case plans using a client centered approach
  • Experience supporting and referring clients to appropriate resources in the community
  • Cultural competency and experience working with diverse populations (including cultural diversity vulnerable and marginalized populations different ages and abilities poverty LGBTQ2 trauma etc.); approach work and others though a lens of equity diversity and inclusion
  • Excellent communication and relationship building skills
  • Strong time management and organizational skills
  • Demonstrated ability to work independently as well as part of a collaborative team
