What Youll Do

Provide skilled nursing care in patients homes
Complete assessments and develop individualized care plans
Educate patients and families on disease management and care needs
Coordinate care with physicians caregivers and interdisciplinary teams
Monitor patient progress and adjust care plans as needed
Build meaningful relationships while improving patient outcomes

What Youll Need

Active RN license in the state applying for
Minimum of 1 year nursing experience
Strong clinical assessment and communication skills
Reliable transportation valid drivers license and auto insurance
Ability to travel throughout assigned territory

Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment

Preferred Qualifications

Home health or hospice experience
Experience with HomeCare HomeBase
