Position Title: Licensed Practical Nurse (LPN)
Location: Falmouth Massachusetts 02540
Weekly Hours: 40 Hours
Schedule: Evening Shift 3:00 PM 11:30 PM Every Other Weekend Rotating Holidays
Duration: 13 Weeks (Strong Possibility of Extension or Permanent Placement)
Position Overview
Navitas Healthcare LLC is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a long-term care team in Falmouth Massachusetts. The LPN will provide direct nursing care to residents supervise support staff and help ensure high-quality patient outcomes in compliance with healthcare regulations and facility standards.
Key Responsibilities
- Provide direct patient care and administer medications as prescribed
- Supervise and support nursing assistants and other care staff
- Maintain accurate documentation for admissions transfers and discharges
- Implement physician orders and individualized care plans
- Monitor patient conditions and document treatment responses
- Perform clinical procedures including wound care catheterization tube feeding and specimen collection
- Collaborate with physicians and interdisciplinary healthcare teams
- Ensure proper availability and management of medical supplies and medications
- Educate residents and families regarding healthcare needs and treatment plans
- Participate in audits inspections and compliance-related activities
Required Qualifications
- Strong understanding of nursing practices patient care procedures and healthcare regulations
- Ability to supervise staff and effectively manage resident care
- Excellent communication and interpersonal skills
- Ability to work independently and make sound clinical decisions
Education
- Graduate of an accredited nursing program or approved Licensed Practical Nursing (LPN) program
Licensure & Certifications
- Active and unrestricted Massachusetts LPN License
- Current BLS / CPR / AED Certification
Preferred Experience
- Prior experience in long-term care rehabilitation or skilled nursing settings preferred
- Strong documentation and charting skills
- Ability to communicate effectively in English both written and verbal
- Ability to meet physical requirements including lifting up to 50 lbs
