The Primary Care Physician will provide comprehensive care to patients across the lifespan including preventive services chronic disease management and acute care. This role emphasizes continuity of care evidence-based practice and collaboration with a multidisciplinary team in a supportive clinical environment.

Key Responsibilities:

• Provide comprehensive primary care services including physical exams diagnosis and treatment planning
• Manage acute and chronic conditions such as diabetes hypertension and respiratory illnesses
• Perform routine procedures and preventive screenings
• Prescribe medications and therapies in accordance with state regulations
• Collaborate with specialists and care teams for coordinated patient care
• Educate patients on health maintenance disease prevention and lifestyle modifications
• Maintain accurate and timely documentation within the EMR
• Participate in quality improvement initiatives and population health programs
