As a Home Health Physical Therapy Assistant heres a big-picture view of what youll do:
- Provide home-based physical therapy to patients who have lost mobility strength and functionality due to an illness or injury
- Work as part of a home health team which may include an RN LPN CNA HHA OT and SLP focused on the patients plan of care and goals
- Participates in ongoing interdisciplinary assessment of the patient.
- Actsat the direction and under the supervision of the treating physical therapist and in accordance with the plan of care.
Participates in the development and evaluation of the plan of care in partnership with the patient representative (if any) and caregiver(s).
Provides services that are ordered by the physician as indicated in the plan of care.
Assiststhe treatingphysical therapist indocumenting apatients progress and response totreatment.
- Counselsand educatesthe patient and thefamilyand other caregivers to promote patient progresstoward mutually established goalsand in meeting healthcare related needs. .
A few must-haves for Home Health Physical Therapy Assistants:
- Current licensure registration or certification as a physical therapistassistant if applicablein the state(s) in which he or she will practice.
- In the last five (5) years two (2) years of experience as a PTA that is relevant to the patient population assigned preferred.
- CPR certification
- Hep B and TB immunizations
- Knowledge of state and federal home health regulations
- Good clinical judgement strong interpersonal skills dexterity and compassion
