Job Summary
Nurse Coordinator Autoimmune Liver Disease (AILD) plays an important role in supporting patients living with autoimmune liver diseases such as autoimmune hepatitis primary biliary cholangitis and primary sclerosing cholangitis. This role focuses on coordinating care educating patients and families and helping patients navigate often complex long-term treatment plans. This is a great opportunity to make a meaningful impact in hepatology by improving patient outcomes and supporting individuals through every stage of their care.
We are looking for someone with strong clinical knowledge excellent communication skills and a genuine passion for patient care.
Duties
- Educate patients and their families about their condition medications (including immunosuppressive therapy) and what to expect over time
- Support patients in managing their treatment plans and monitoring for side effects
- Help connect patients with resources and support services
- Keep clear and up-to-date documentation of patient interactions and care plans in EPIC
- Work closely with lead hepatologist and the broader care team to ensure smooth coordinated care
- Take part in quality improvement initiatives and program development
- Contribute to the academic mission of the autoimmune liver disease clinic by helping patients understand the importance of and participation in approved research studies
Qualifications :
- Completion of a university college or other approved nursing program
- Current Basic Cardiac Life Support (B.C.L.S.) program Level II: Basic Rescuer certification
- Current certificate of competency from the College of Nurses of Ontario
- Physical Health Assessment Course preferred
- Completion of the ECG interpretation course is an asset
- Minimum of 2 years of clinical experience in hepatology gastroenterology or a related field.
- Certification in hepatology nursing or related specialty (preferred).
- Experience with phlebotomy IV insertion insertion of and access to vascular access lines and administration of blood products
- Demonstrated ability to exercise initiative and good judgement with the ability to multi-task
- Ability to work as an integral member of a team
- Excellent independent decision-making and problem-solving skills
- Excellent organizational and time management skills
- Excellent verbal interpersonal and written communication skills
- Patient teaching skills
- Ability to work effectively as a member of a multidisciplinary team
