It is responsible for safeguarding and preserving lives and property through prevention protection and preparation as well as the care custody and supervision of all adults sentenced for violating North Carolina laws. This department includes state prisons with custody healthcare and facility operations staff as well as probation/parole officers who supervised sentenced people in the community. Description of Work

The Social Worker serves as a key member of a multidisciplinary treatment team within the Division of Adult Correction. The role focuses on assessing the needs of offenders preparing for release particularly those with serious mental illnesses or developmental disabilities. The Social Worker helps ensure continuity of care and supports public safety by connecting individuals with necessary treatment and services in the community.

Job Duties include:

Conduct assessments for post-release needs including medical care mental health treatment and developmental disability services.
Participate in treatment planning for individuals with severe mental illness.
Develop community resource connections and referrals for aftercare services.
Identify and address the needs of special populations.
Provide assessments and recommendations for ADA-related referrals.
Complete Medicaid applications for offenders referred by program managers.
Work across multiple correctional facilities (ranging from minimum to close custody) as needed.
Support stabilization of mental illnesses during incarceration and encourage continuation of treatment after release.

The Rehabilitation and Reentry Division oversees services programs and community partnerships aimed at helping individuals reintegrate into theircommunitiesfollowing incarceration. Staff members work with people who are incarcerated probationers post-release supervisees and parolees. This division collaborates with prison and supervision staff to create individualized reentry plans coordinate services and deliver evidence-based programs to improve outcomes and reduce recidivism.
