Physical Therapist (PT) Skilled Nursing Facility
Location: Franklin PA
Assignment Length: 13 Weeks
Schedule: Day Shift 8-Hour Shifts
Weekly Hours: 40 Hours
Position Overview
Navitas Healthcare LLC is seeking a dedicated and compassionate Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing Facility (SNF) setting. The PT will provide rehabilitation services focused on improving mobility strength and overall functional independence for long-term care residents.
This role is fully facility-based with no outpatient responsibilities.
Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Deliver skilled therapeutic interventions to improve mobility and function
- Monitor patient progress and modify treatment plans as needed
- Educate patients and caregivers on therapeutic exercises and mobility strategies
- Collaborate with interdisciplinary healthcare teams to ensure quality patient care
- Maintain accurate and timely documentation
Qualifications
- Active Physical Therapist (PT) license
- Prior experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) settings preferred
- Strong clinical assessment and patient care skills
- Ability to work independently and collaboratively within a team environment
- Excellent communication and interpersonal skills
