About the Role

An opportunity currently exists for an experienced clinician (Registered Nurse or Allied Health Professional) to join the Health Monitor Team on a permanent part time basis (0.4 FTE).

The Health Monitor clinician will provide phoned based assessment monitoring short term care coordination and onward referral of clients that are recently discharged from hospital. Management strategies will be targeted to prevent avoidable hospital presentations.

As an experienced clinician the Health Monitor will demonstrate knowledge in the management of clients with a diverse range of issues both medical and psychosocial needs and be experienced in the coordination of care post discharge from the hospital setting.

This role will work with the team to deliver high quality phone-based care to clients that have been recently discharged from acute services. This position will require weekend work.

Classification for this position will be Registered Nurse YY4 / Allied Health Grade 2.

Duties

Direct clinical provision of high quality and evidence-based clinical assessment intervention education and management.

Undertake phone-based assessments in collaboration with the client to determine needs post discharge.

Use remote monitoring technology to triage and manage care pathways

Collaborative short term care planning and care coordination to address immediate health and wellbeing needs.

Monitor health outcomes and plan for the transition and exit from the program.

Participate in quality initiatives team processes and service development activities

Your Contribution

Nursing or Allied Health qualification and registration with Australian Health Practitioner Regulation Agency (AHPRA).

Minimum 2 years experience in relevant acute/community setting

Comprehensive assessment clinical management skills and experience with discharge planning and liaising with other service providers

Experience working with clients with multiple health issues and complex care needs.

Ability and willingness to work in a phone based model of care and on weekends.

Excellent organisational coordination and time management skills.

About the Health Monitor team

This position is part of our St Vincents Care at Home division which provides the opportunity for more people to receive high quality clinical care at home. Integrated with St Vincents Health Australia hospital and community care network we are a multidisciplinary team of nursing and allied health supporting individuals following discharge from hospital. We are an experienced supportive and dynamic team committed to quality virtual care coordination.

Health Monitor is a virtually enabled care coordination service providing monitoring and management of individuals at risk in the community. The service uses interactions via remote monitoring and telephone to identify and address issues that can lead to hospital representations including health literacy referrals to community supports medication management symptom management carer stress and other contributors to safe and effective discharge from hospital.
