Home Health Clinical Success Mentor
Supporting Des Plaines and Tinley Park Branches
At Elara Caring were looking for a highly experienced Skilled Home Health RN who is ready to step into a clinical leadership and mentorship roleguiding new clinicians strengthening preceptors and elevating care delivery across our branches.
This is not a role for someone new to home health. This is for the nurse others already turn to for guidancethe one who knows OASIS inside and out understands the realities of the field and is passionate about developing others.

Your Impact
As a Clinical Success Mentor you will bridge the gap between training and real-world field success:
Lead & Elevate New Hire Success
Guide clinicians through hands-on field orientation ensuring confidence in:
Start of Care (SOC) ROC Recertifications and routine visits
Real-time clinical decision-making in the home setting
Partner closely with preceptors and leadership to ensure a consistent high-quality onboarding experience
Evaluate readiness provide feedback and support clinicians as they transition to independence
Drive OASIS & Documentation Excellence
Coach clinicians on accurate compliant documentation
Reinforce best practices aligned with Medicare state and regulatory standards
Improve outcomes tied to VBP HHCAHPS and hospitalization rates
Develop Preceptors & Clinical Culture
Mentor and support preceptors strengthening their ability to teach and lead
Foster a culture of accountability learning and clinical excellence
Identify skill gaps and deliver targeted education and coaching
Support Ongoing Education & Operations
Collaborate with leadership to implement training initiatives and process improvements
Facilitate skills labs competency validations and ongoing education
Act as a clinical resource and trusted advisor across branches
What Were Looking For
Were seeking a true home health expert who thrives in a mentorship role:
Registered Nurse (RN) with active unrestricted license (multi-state/compact preferred)
3 years of Skilled Home Health experience strongly preferred (not suitable for candidates without a strong SHH background)
Deep expertise in:
OASIS accuracy and documentation
Medicare regulations and home health compliance
End-to-end home health workflows (SOC through discharge)
Proven experience as a preceptor mentor or informal clinical leader
Strong ability to teach coach and influence clinicians in the field
Comfortable working across multiple locations in a fast-paced evolving environment
Willingness to travel 50% within the region - need to live in the Northeast to help support the branches

The base salary for this position is$90000 to $95000 annually based on the companys good faithestimate at the time of posting
