JOB SUMMARY

Provides direct client services by screening for program eligibility maintaining close contact with clients coordinating recovery support services and linking them with community resources.

MINIMUM QUALIFICATIONS

Bachelors degree from an accredited college or university in Social Work Psychology Sociology Social Sciences Criminal Justice or an equivalent combination of related education and experience.

Due to the nature of this position the successful applicant must pass a required background check through fingerprinting and enrollment in the continuous RAP Back (Record of Arrest and Prosecution Back) program in accordance with current County Human Resources policy requirements.

ESSENTIAL FUNCTIONS

  • Screens clients for eligibility orients clients to the program assists clients in establishing their recovery service plan and conserves the clients limited benefit by ensuring proper budgeting of funds.
  • Maintains current client contact information and schedules at least bi-monthly contact with the client through face-to-face or electronic means.
  • Supports clients in making informed independent choice of service providers.
  • Acts as a client advocate.
  • Links clients with existing community resources and tracks use of services authorized through vouchers; extends or shortens voucher authorization to maximize their recovery capital.
  • Monitors the effectiveness of recovery support services; verifies that authorized services are being utilized; authorizes additional services as needed; ascertains client satisfaction; documents case record; and continues to evaluateif services are appropriate.
  • Communicates client progress needs and program engagement statuswith treatment providers and agencies parole and probation officers case managers and other community entities.
  • Conducts outreach to community agencies to develop partnerships in order to increase program referrals and efficiency.
  • Facilitates and attends meetings with community partners to discuss program progress developstrategies to improve and expand program and coordinate services across agencies.
  • Ensures compliance with federal state and county grant requirements.
  • Trains providers to accept and bill for vouchers in the electronic voucher management system.
  • Reviews billing records case management notes and logs for discrepancies.
  • Performs administrative tasks such as filing answering phone and greeting clients and visitors.

  • Utilize relevant technology including all aspects of the voucher management system

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS

Must be able to sit in an office setting for long periods of time and be able to meet clients in a variety of community setting for case management.
