ACTT Team Leader (LCSW / LCMHC / LMFT)

Schedule: MonFri 8:005:00
On-call: 24/7 rotation shared across 6-8
What this job looks like (week to week)
- Caseload: 812 adults with serious mental illness (SMI) substance use
- Field work in homes shelters hospitals
- Office/admin time for supervisions documentation etc.
- Team: psychiatry nursing therapists peer support
- Crisis calls: part of rotating 24/7 coverage
What you do
Clinical- Complete biopsychosocial substance use assessments
- Build treatment plans with the ACTT team
- Deliver therapy using:
- Motivational Interviewing
- Cognitive Behavioral Therapy
- Relapse prevention
- See clients in homes shelters ERs and community locations
- Run 24 person-centered meetings/week
- Support stabilization (med adherence coping skills anger regulation)
- Respond to behavioral health crises in the community
- De-escalate situations
- Guide 68 team members on clinical decisions
- Coordinate with psychiatry nurses employment specialists peer support
- Review documentation to meet Medicaid ACTT standards
Requirements
License (North Carolina)- LCSW LCMHC or LMFT
- 1 year working with adults (paid or volunteer)
- Direct work with SMI substance use or crisis populations
- Drivers license daily travel
- Lift up to 25 lbs
