Your Role as a Social Services Coordinator:

• Provide social services support to residents addressingpsychosocial emotional and environmental needCommunicate with residents and families to resolve concerns provide guidance and ensure needs are met
• Complete and maintain comprehensive assessments including social histories care plans and progress notes
• Advocate for residents rights ensuring dignity privacy and respect at all times
• Collaborate with interdisciplinary teams including staff consultants and healthcare professionals
• Maintain strict confidentiality of resident information in accordance withHIPAA regulations
• Support a calm caring and respectful environment for residents and staff
• Participate in care planning and contribute to individualized resident care strategies
• Attend requiredtrainings in-services and meetings

What Were Looking For:

• Bachelors degree in Social Work or related field from an accredited college or university
• Current Social Worker license in the State of Ohio
• Minimum of 2 years of experience in a healthcare setting preferred
• Strong problem-solving organizational and multitasking skills
• Excellent communication and customer service abilities
• Ability to maintain accurate and timely documentation
• Ability to work both independently and collaboratively as part of a team
• Compassionate calm and professional demeanor in emotionally demanding situations
• Ability to read write and speak English
