Key Responsibilities

• Build and maintain referral relationships with physicians hospitals skilled nursing facilities and community partners
• Educate referral sources and the community on hospice services and eligibility
• Manage and grow a designated territory through strategic outreach and partnership development
• Collaborate with intake admissions and clinical teams to ensure a seamless patient onboarding process
• Track outreach activity and performance using CRM systems and territory planning tools
• Contribute to a high-performing ethical sales culture centered on patient outcomes

Qualifications

• Experience in healthcare sales
• Hospice home health or post-acute care experience strongly preferred
• Demonstrated success in sales performance and relationship building
• Established network within the Philadelphia healthcare market preferred
• Strong communication presentation and interpersonal skills
• Experience managing a territory and utilizing CRM tools
