Position Summary
We are seeking dependable and experienced Licensed Practical Nurses (LPNs) to deliver high-quality compassionate care in a Long-Term Care (LTC) setting. The ideal candidate is flexible reliable and comfortable working variable shifts while maintaining strong clinical standards.
Key Responsibilities
Provide direct nursing care to residents in a long-term care environment
Administer medications and treatments as prescribed
Monitor patient conditions and promptly report changes to the RN or physician
Assist residents with activities of daily living while ensuring comfort and safety
Accurately document patient information using an electronic medical records (EMR) system
Collaborate with interdisciplinary healthcare team members to support optimal patient outcomes
Adhere to all facility policies procedures and safety standards
Required Qualifications
Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
Pennsylvania Department of Aging Fingerprint Clearance Letter (Required)
Minimum 1 year of experience in long-term care or a nursing home setting preferred
Ability to work flexible shifts (day evening and night)
Strong clinical organizational and communication skills
Reliable punctual and team-oriented
Preferred Qualifications
Prior experience in a long-term care or skilled nursing facility
Familiarity with electronic charting systems (e.g. Matrix or similar EMR platforms)
