Job Title: Licensed Practical Nurse (LPN)

Position Summary
We are seeking dependable and experienced Licensed Practical Nurses (LPNs) to deliver high-quality compassionate care in a Long-Term Care (LTC) setting. The ideal candidate is flexible reliable and comfortable working variable shifts while maintaining strong clinical standards.
Key Responsibilities
- Provide direct nursing care to residents in a long-term care environment
- Administer medications and treatments as prescribed
- Monitor patient conditions and promptly report changes to the RN or physician
- Assist residents with activities of daily living while ensuring comfort and safety
- Accurately document patient information using an electronic medical records (EMR) system
- Collaborate with interdisciplinary healthcare team members to support optimal patient outcomes
- Adhere to all facility policies procedures and safety standards
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
- Pennsylvania Department of Aging Fingerprint Clearance Letter (Required)
- Minimum 1 year of experience in long-term care or a nursing home setting preferred
- Ability to work flexible shifts (day evening and night)
- Strong clinical organizational and communication skills
- Reliable punctual and team-oriented
Preferred Qualifications
- Prior experience in a long-term care or skilled nursing facility
- Familiarity with electronic charting systems (e.g. Matrix or similar EMR platforms)
