Job Summary
The Service Coordinator is responsible for partnering with clients in recovery from mental illness chemical dependency and/or co-occurring conditions to identify access and maintain linkages with treatment housing rehabilitation and community services. The goal is to help individuals maintain stable housing in the community and improve their overall quality of life. Working within a person-centered approach the Service Coordinator teaches clients essential living skills for independent living.
In compliance with NYSs Pay Transparency Act the salary range for this position is listed above. East House considers factors such as (but not limited to) scope and responsibilities of the position candidates work experience education/training key skills internal peer equity as well as market and organizational considerations when extending an offer. The hiring process for this position may require a criminal background check and/or motor vehicle records check. . Partner with clients to develop individualized service plans based on client-identified goals.
Coordinate and advocate for access to mental health substance use physical health financial and other community resources.
Provide support in the community and in client homes utilizing clinical knowledge and sound judgment.
Participate in case reviews and planning to ensure quality and stability of client services.
Support client access to housing including landlord relations lease retention and financial assistance.
Assist clients with benefit applications budgeting and payee agency coordination.
Promote family well-being through school recreation and healthcare connections.
Provide medication support as required by the program.
Maintain documentation in accordance with funder and agency standards.

Qualifications
Education & Experience
High School Diploma or equivalent and 4-6 years of human services experience OR
Associate degree and 2-4 years of experience OR
Bachelors degree in human services and 1-3 years of experience.
Licenses/Certifications
Valid NYS drivers license in good standing and access to a personal vehicle for work-related tasks.
Skills & Abilities
Knowledge and experience supporting culturally diverse individuals with psychiatric or chemical dependency concerns.
Skills in counseling crisis assessment and intervention communication and client monitoring.
Proficiency in Microsoft Word basic computer literacy and database usage.

Working Conditions/Work Environment
Traditional office or household environment with occasional evening or weekend hours.
Frequent travel and client transport required.
Physical Demands
Requires sitting handling paperwork using a computer and effective verbal/written
communication.
Occasional lifting (up to 30 lbs.) walking standing stair climbing and travel.
Visual and auditory acuity for normal office tasks.
