Job Description
Position Summary: The Ambulatory Care Clinical Pharmacist will provide ambulatory pharmaceutical care services using approved protocols in collaboration with other health professionals including managing patient drug therapy and providing patient education while promoting high quality evidence-based cost-effective pharmaceutical care. Independent thinking and decision-making is required. Performs these duties in the clinical areas assigned. Full pharmacotherapy knowledge.
Schedule: Mon-Fri (Flexible hours beetween 8 am - 5:30 pm)
Hybrid Role: Tuesdays & Wednesdays Onsite
Essential Job functions:
- Provides Medication Therapy Management (MTM) services under an approved NJ State Board of Pharmacy Collaborative Practice Agreement with primary care providers in sites as assigned.
- Promotes rational evidence based and cost-effective outpatient drug therapy through close participation in the drug therapy decision-making process with the care team.
- Under Collaborative Practice protocols initiates adjusts monitors and recommends changes in drug therapy based on information obtained through patient interviews objective monitoring parameters review of drug profiles and the patient medical record.
- Optimizes medication regimens and clinical outcomes in patients with diabetes hypertension heart failure dyslipidemia arrhythmias angina thromboembolism asthma COPD renal disease and other high risk patients as required.
- Optimizes utilization of cost saving generic alternatives across all patient encounters.
- Optimizes safe medication use by reducing utilization of inappropriate medications in patients 65 and older.
- Optimizes patient medication adherence through counseling and application of motivational interview skills.
- Designs implements and monitors therapeutic drug plans to achieve improved outcomes through both direct and telephonic interactions with patients and providers in assigned areas.
- Knowledge of all Meds not limited to Oncology/Infusion.
- Reviews patient medication regimens for clinical effectiveness drug selection dosing contraindications side effects potential drug interactions as required.
- Conducts patient medication history inquiries and drug information requests upon referral from physicians and other medical personnel.
- Analyzes and employs a systematic efficient and thorough procedure for retrieving drug information.
- Formulates response and follows up consistently on drug information recommendations and assesses effectiveness.
- Collaborate with other health care professionals to plan monitor review or evaluate the quality or effectiveness of drugs or drug regimens providing advice on drug applications or characteristics.
- Communicates and documents findings with prescribers and provides appropriate alternatives to current treatment plans as needed in the EHR.
- Identifies resolves and prevents potential and actual medication-related problems including: untreated indications improper drug selection sub-therapeutic dosage overdose failure to receive medication adverse drug reactions drug interactions and medication without indication.
- Reviews and updates Collaborative Practice Agreements annually.
- Coordinates and interprets laboratory monitoring of drug therapy including serum drug levels electrolytes and other approved drug-related laboratory testing.
- Participates on the P&T committee and quality improvement committees and effectively communicates with stakeholders.
- Participates in the review and analysis of Pharmacy Data reports with objective of identifying actionable opportunities to improve patient outcomes.
General Job functions:
- Develops and participates in operational and clinical projects as determined by current departmental needs and ongoing projects.
- Participates in the development and presentation of education to providers and staff via department meetings newsletters formal utilization reviews and one-on-one interactions
- Assures ongoing performance improvement of the pharmaceutical care services provided.
- Provides feedback and/or consultation related to technology utilization for documentation of MTM clinical pharmacy outcomes or other identified medication related issues
- Offer health promotion and prevention activities for example training people to use devices such as blood pressure or diabetes monitors.
- Other job duties as required.
Physical Job Requirements:
- Physical mobility which includes movement from place to place on the job taking distance and speed into account
- Dexterity of hands and fingers
- Endurance (e.g. continuous typing prolonged standing/bending walking)
- Balance is maintained during climbing bending and/or reaching
Education Certification Computer and Training Requirements:
- Pharm D required.
- PGY-1 Residency required.
- NJ State Board of Pharmacy licensed required.
- Board of Pharmaceutical Specialties Certification: Board Certified Ambulatory Care or Pharmacotherapeutics Pharmacist required.
- American Pharmaceutical Association MTM Certification preferred.
- New Jersey State Board of Pharmacy Collaborative Practice Agreement required. (If you are hired without NJ CPA it is a requirement that you obtain the NJCPA within 4 months from the end of your introductory period. Should you not obtain the NJCPA you may be subject to termination of employment ).
- Ability to communicate in English both orally and in writing required.
- Experience with Standard Office Equipment (Phone Fax Copy Machine Scanner Email/Voice Mail).
- Experience with Standard Office Technology in a Window based environment required.
- Prior Clinical Pharmacy experience: 2 to 4 years minimum 5 years preferred
- Adheres to local state and federal laws regarding the profession of pharmacy practice.
Travel Need to travel to other company or client locations at a minimum 20% of the time.
