Primary Function/General Purpose of Position
The Community HealthWorker focuseson direct support outreach and engagement to improve community health outcomes focusing on improving health well-being and access to essential services.TheCommunity Health Workerservesasaliaison betweencommunity membersor patientsand health and social service systems providing culturallyappropriate healtheducation and addressing basic socialdriversof health.Community Health Workers may work ina variety of settings includingserving in the community orassistingpatients in physician practicesbased onspecificmarketdesign.
Essential Job Functions
Builds trusting relationships with individuals and communities to mediate betweenindividualsand health/social service systems.Identifiesand engages individuals and families in need through outreach activities and community events.
Evaluates the specific needs and challenges faced by community members including health social and educational needs.Identifiesbarriers such as housing food insecurity and transportation challenges and connectsindividualstoappropriate resources.
May conduct outreach activities including home visits and participation in community health fairs and local eventsto build awareness and engageunderserved populations.
Maintainsaccurateand up-to-date records to support advocacy effortsmonitoroutcomes and contribute to program evaluation.
May perform orassistwithhealth education and/orbasic health screenings such as blood pressure and glucose checks underappropriate supervision.
Provides support toongoing communityhealthprograms.

Licensing/Certification
Community Health Worker (CHW) Certification in the state of practice (preferred;within 12 months of hire)
Valid drivers license with good driving record (required South Carolina only)
Education
High School Diploma or equivalent (required)
Bachelors degree in Social Work Public Health Psychology or related field(preferred)
Work Experience
1year of recent experience in a community outreachor healthcaresetting (preferred)
Training
None
Language
Bilingual based on market needs (preferred)
