Job Summary

Primary Responsibilities (including but not limited to):

  • Complete all documentation for assessments observations and planning evaluation pertaining to clients.
  • Re-evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan.
  • Utilize professional assessment skills to identify physical mental and psycho social needs of clients and update care plans to meet these needs.
  • Responsible for caregiver oversight and competency adherence to AHCA standards.
  • Function as a resource and mentor peers and subordinates to maintain professional LPN standards.
  • Act on clients behalf to assure patient rights.
  • Must be able to rotate on call including weekends with nursing staff.
  • Call caregivers from the daily update email when its your day on call write up caregiver report and turn into the office.
  • Be aware of home safety issues strive to accomplish all responsibilities in a safe manner and consistently reinforce good safety attitudes and practices displayed by other staff members.
  • Respond promptly to any client complaints concerning a caregiver document complaints review complaints with the scheduling team and report to Nurse and office.
  • Document daily / weekly agenda on the office shared calendar in advance and update when necessary.
  • Work with D.O.N. on Monthly and or quarterly mandated reports.
  • Visit each client caregivers and Floaters in the Preferred Provider buildings weekly document updates on computers and on weekly nursing summary.
  • Visit clients that are in the hospital or rehabilitation bring a blanket or teddy bear and speak with their nursing staff for updates where possible.
  • Med management clients must be seen every 2 weeks or twice a month to fill in pill planners or medication dispensers call in any necessary refills at the time of the visit. verification of medications must be submitted to the doctor every 60 days for signature.
  • Report for mandatory staff meeting EVERY Monday at 10 am in the office.
  • Respond to emails and texts within a timely manner especially while on call.
  • Report to direct supervisor for any clarifications on tasks assigned.
  • Fill out annual evaluations and 3-year competencies for caregivers have caregivers sign off and return to HR.
