Title: Physical Therapist
Location: SNF (Skilled Nursing Facility)
Salary Range: $41-$43.50 per hour
Hours: 35 hours per week Monday through Friday with weekends as needed
Key Responsibilities:
- Perform initial evaluations and assessments of patients physical abilities and limitations
- Develop individualized treatment plans based on patients needs and goals
- Provide hands-on therapy to patients using a variety of techniques and modalities
- Monitor patients progress and adjust treatment plans as needed
- Educate patients and their families on exercises and techniques to improve mobility and function
- Collaborate with other healthcare professionals to ensure comprehensive care for patients
- Maintain accurate and up-to-date patient records
- Follow all safety and infection control procedures
- May be required to float to other facilities as needed
Qualifications:
- Bachelors Masters or Doctorate degree in Physical Therapy from an accredited program
- Current state license to practice as a Physical Therapist
- Minimum of 2 years of experience in a SNF or similar setting preferred
- Strong knowledge of physical therapy principles and techniques
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work weekends as needed
- Ability to float to other facilities as needed
