Job Summary
Job Title: Physical Therapist
Salary Range: $41-45 per hour
Position Type: Full-Time Monday-Friday with weekend rotation
Facility: SNF Community offering post-hospital short-term long-term and respite care
Number of Beds: 181 SNF beds
Responsibilities:
- Conduct initial evaluations and assessments to determine patients needs and develop individualized treatment plans
- Provide hands-on therapy and exercises to improve patients strength mobility and range of motion
- Educate patients and their families on exercises and techniques to continue their rehabilitation at home
- Monitor and document patients progress and adjust treatment plans as needed
- Collaborate with other healthcare professionals including doctors and nurses to ensure comprehensive care for patients
- Communicate effectively with patients families and caregivers to provide support and guidance throughout the rehabilitation process
- Adhere to all legal and ethical standards in patient care and documentation
- Participate in continuing education and professional development opportunities to stay current with best practices and advancements in the field of physical therapy
Requirements:
- Bachelors Masters or Doctorate degree in Physical Therapy from an accredited program
- Current state license to practice as a Physical Therapist
- Minimum of 1-2 years of experience in a similar setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
- Knowledge of and adherence to all legal and ethical standards in patient care
