Job Description: We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care/skilled nursing facility. The LPN will be responsible for delivering direct patient care supervising nursing assistants and ensuring residents receive compassionate and effective treatment in accordance with care plans and facility standards.
Responsibilities:
Provide direct nursing care to residents
Supervise and support nursing assistants in daily care activities
Administer medications and treatments as prescribed
Perform clinical procedures such as catheterization tube feeding and wound care
Transcribe physician orders and maintain accurate documentation
Monitor resident conditions and report changes to physicians
Educate residents and families on health care needs
Participate in facility surveys and ensure regulatory compliance
Requirements:
Graduate of an accredited nursing program or approved LPN program
Active unencumbered Massachusetts LPN license
BLS certification (American Heart Association required)
Strong clinical leadership and communication skills
Ability to make independent decisions and work in a team environment
Knowledge of nursing practices procedures and long-term care regulations
Ability to lift up to 50 pounds
