We are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents lives in a rewarding setting.

Position Summary:
As a PRN Physical Therapist you will evaluate and treat residents to improve their mobility strength and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality individualized care.

Key Responsibilities:

• Perform comprehensive evaluations of residents physical abilities and needs.
• Develop and implement personalized treatment plans to improve strength mobility balance and overall function.
• Provide therapeutic interventions including exercises manual therapy and training on assistive devices.
• Monitor residents progress and modify therapy plans as needed to achieve goals.
• Collaborate with an interdisciplinary team to ensure holistic care.
• Educate residents and their families on therapy goals exercises and strategies for continued improvement.
• Accurately document evaluations treatment sessions progress notes and discharge plans in compliance with regulatory and facility standards.
• Participate in quality improvement initiatives and contribute to the rehabilitation departments success.

Qualifications:

• Doctorate or Bachelors/Masters degree in Physical Therapy from an accredited program.
• Current and valid Physical Therapist license in the state of Colorado.
• Experience in skilled nursing or rehabilitation settings is preferred.
• Strong communication and teamwork skills with a resident-centered approach.
• Ability to work independently and adapt to a flexible PRN schedule.
• Dedication to providing compassionate and effective therapy services.
