Are you a Registered Nurse looking for a high-impact long-term assignment at one of the nations top-ranked hospitals in partnership with DaVita DaVitas Patient Pathways team is seeking a Shining Star for a contract role at Cedars-Sinai Medical Center.
This position is perfect for an RN who thrives on autonomy and clinical coordination. You will serve as the primary link between patients families and elite physician teams ensuring that patients with chronic kidney disease (CKD/ESRD) receive seamless high-quality care.

What Youll Do
On-Site Coordination: Work directly within the hospital to facilitate quality cost-effective care transitions for renal patients.
Reduce Fragmentation: Act as a dedicated advocate to ensure continuity of care between the hospital the family and the outpatient provider.
Performance Coaching: Apply CQI techniques and clinical data to drive better patient satisfaction and health outcomes.
What You Bring (The Shining Star Profile)
Licensure: Current good-standing CA Registered Nurse (RN) license.
Experience: Minimum of 3 years in clinical nursing (Required).
Leadership: Background in healthcare performance coaching or acting as a change agent.
Preferred: Renal nursing experience BSN or certifications like CNN or CCM.
Logistics: Valid CA drivers license with a positive record.
