About The Position:

Join Oswego County Opportunities (OCO) as a Family Planning Advocate and make a meaningful impact supporting youth adults and families across your
this dynamic field-based role youll provide supportive counseling education and hands-on advocacy while connecting individuals to essential reproductive and overall
health services. Youll lead engaging community-based education on various sexual health topics while also coordinating care conducting assessments and guiding clients
toward positive outcomes. This position is ideal for someone who is compassionate adaptable and motivated to work both independently and collaborativelybuilding
strong partnerships with schools providers and community organizations to empower individuals and improve health outcomes across diverse populations.

Job Summary:

Provides direct services to people of all ages according to OCO Universal Service Delivery methods in diverse locations including OCO offices other agency offices program sites public places and the homes of consumers. Independently provides early intervention and care management services to an assigned population and oversees project functions as assigned. Carries out duties in accordance with Agency and Program policies and procedures. Responsible for implementing community and school-based education initiatives that support reproductive health services and promote positive health outcomes.

JOB DUTIES AND RESPONSIBILITIES:

  • Provides crisis intervention supportive counseling advocacy and solution focused decision making for children youth and families.
  • Works with individuals seeking reproductive health service experiencing sexual health issues and/or seeking other health services.
  • Provides connection to unintended pregnancy prevention related health care services and basic information about the Family Planning Program
  • Ensures safe and discreet connection to needed pregnancy prevention resources such as condoms birth control etc.
  • Responds to consumers who are contacting in person through a variety of multimedia avenues and on the phone to provide information about services & the status of their services
  • Works as part of a team to provide home visits assessments and education to increase health outcomes provide services support and follow-up.
  • Advocates accompanies and/or negotiates on behalf of consumers.
  • Provide trainings and Evidence Based Programs to youth & community partners on comprehensive sexual health topics including: Adolescent Pregnancy Prevention Trauma Informed Care LGBTQ Inclusivity STD/HIV & Hepatitis prevention PrEP & PEP and harm reduction.
  • Works cooperatively with schools community members agencies and other service providers.
  • Informs consumers of available services in the community and assists with the referral of primary and secondary consumers to services and monitors the provision of services to all consumers.
  • Develops and maintains assessments individualized service plans and service coordination activities for children youth and families.
  • Provides the teaching and coaching of life skills; facilitates education and support groups as assigned.
  • Provides or arranges transportation to enable consumers to connect with needed community resources.
  • Collaborates with community partners in the development and implementation of joint protocols; to ensure compliance with regulations and eligibility requirements.
  • Create and distribute culturally competent evidence-based educational materials tailored to diverse populations
  • Assists in training program staff and community partners in specialized topic areas as assigned.
  • Attends internal and external meetings as assigned and assists in meeting facilitation as required.
  • Completes all documentation as required including electronic records; documents for billable services as required.

Responsible for:

  • Assists manager in organizing and facilitating partnership meetings to advance the objectives of the project
  • Tracking and compiling statistics related to program activities and outcomes and completing reports for various project stakeholders
  • Staying up to date with best practices in the field and sharing information with project partners and co-workers

Other:

  • Develops and maintains effective working relationships with community partners such as schools medical providers and other services
  • Conducts rapid HIV screening including pre and posttest counseling and assessments
  • Carries out other special projects as assigned.

JOB REQUIREMENTS:

  • Must be able to work independently and be self-motivated resourceful organized and adaptable in carrying out job duties
  • Must be familiar with the geographical layout of the county and have knowledge of human service resources in the county
  • Must have public speaking skills and creativity for making presentations on sensitive issues to various groups.
  • Must have demonstrated ability to work with others in a warm non-judgmental manner and be a positive role model.
  • Must exhibit professionalism good judgment and maintain strict confidentiality with regard to client information.
  • Must exhibit detailed knowledge of regulations services rights and responsibilities for the population served.
  • Must be flexible with work schedule to meet the needs of the program
  • Must have acceptable health to carry out the responsibilities of the position.
  • Must have a valid NYS Drivers license with driving record within policy and access to a properly insured reliable vehicle for regular travel and transporting consumers.

MINIMUM QUALIFICATIONS:

  • Must have a Bachelors Degree in a Human Services area; and
  • Must have 1 to 3 years experience with children youth families or a special population; or
  • Any equivalent combination of education experience and training.
