Job Description:
Licensed Practical Nurse (LPN) responsible for providing patient care in a fast-paced outpatient setting including rooming patients triage and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high-quality patient-centered care.
Responsibilities:
- Room patients and maintain efficient patient flow for scheduled and walk-in visits
- Perform triage (in-person and phone) and prioritize patient needs per protocol
- Obtain and document vital signs medical history and clinical information in the EMR
- Communicate patient concerns and clinical findings to providers
- Assist providers with exams procedures and patient care
- Administer medications immunizations and treatments as ordered
- Process prescription refills and patient calls/messages
- Perform or assist with in-office testing and specimen collection
- Provide patient education and support for care plans and follow-up
- Maintain accurate timely documentation and uphold confidentiality standards
Qualifications:
- Active unrestricted LPN license in Vermont
- Current CPR certification
- Strong clinical assessment and triage skills
- Excellent communication and interpersonal skills
- Ability to multitask and thrive in a fast-paced environment
