About the Role This is a people-first role at a homecare agency where your work directly shapes how clients and families experience care from day one. As the Intake Coordinator youll serve as the first point of contact for potential clients guiding them through assessments managing the full intake pipeline across multiple service types and ensuring every person feels supported and informed throughout the process. Beyond intake youll also play an active role in the agencys growth through outreach and relationship-building with prospective clients and referral sources.
Key Responsibilities
Client Intake & Assessment
Respond promptly to inquiries via phone email and in person regarding homecare services
Conduct initial assessments to determine eligibility and identify client needs
Manage and complete intake procedures for HHA MLTC HMO NHTD and PDN services
Collect verify and maintain accurate client information including medical history insurance details and service preferences
Care Coordination & Communication
Keep clients and families informed throughout the intake process with timely follow-ups and clear communication
Address client concerns and escalate complex cases to the appropriate team members when needed
Provide regular updates to marketing teams on the status and progress of referrals
Outreach & Growth
Contact prospective members to offer and explain homecare services
Support community outreach initiatives and build relationships with referral sources
Drive referrals through the full intake pipeline to secure timely authorizations and convert cases to active services
Qualifications
Strong verbal and written communication skills
Proficiency in computer software and standard office tools
Excellent organizational scheduling and time management abilities
Strong interpersonal skills with a commitment to confidentiality and professionalism
