Licensed Practical Nurse (LPN) Home Care
Location: West Philadelphia PA
Schedule: Contract 40 hours weekly 8am 430pm
Position Overview
The Licensed Practical Nurse delivers high-quality patient-centered nursing care to clients in their homes. Working independently and as part of an interdisciplinary team the LPN supports individuals with acute chronic or post-operative needs promoting health independence and quality of life.
Key Responsibilities
- Provide direct nursing care in patients homes in accordance with individualized care plans
- Administer medications injections and treatments as prescribed
- Perform wound care dressing changes and monitor healing progress
- Assess patient condition and report changes to the RN or care team
- Assist with activities of daily living (ADLs) such as bathing grooming and mobility support
- Educate patients and families on disease management medications and home safety
- Maintain accurate and timely clinical documentation
- Coordinate care with RNs physicians and other healthcare professionals
- Ensure infection prevention and control practices are followed in the home setting
- Travel between patient homes within the community
Qualifications
- Active LPN license in Pennsylvania
- Current BLS (Basic Life Support) certification
- Valid drivers license and reliable vehicle (required for community visits)
- Minimum 2 years of nursing experience preferred
- Previous home care or community health experience is an asset
- Wound care experience
