Job Summary:We are seeking a detail-oriented Virtual Care Coordinator to support telehealth and remote patient monitoring initiatives within a dynamic healthcare environment. This role involves coordinating virtual care services monitoring patient status remotely and ensuring high-quality patient-centered care through effective communication and collaboration with clinical and technical teams.
Key Responsibilities:
- Monitor patients remotely for safety and clinical needs
- Support hospital-based telemedicine services and virtual care workflows
- Assist in implementing telehealth and remote patient monitoring programs
- Collaborate with clinical teams leadership and technical partners
- Communicate effectively with patients providers and support teams
- Monitor hospital environmental status through virtual systems
- Assist in training coordination and telehealth best practice implementation
- Maintain accurate documentation in EMR systems
- Ensure adherence to quality safety and compliance standards
Required Qualifications:
- Minimum 2 years of telehealth experience (required)
- Experience in direct patient care
- Strong computer skills and familiarity with EMR systems
- Excellent organizational and multitasking abilities
- Strong attention to detail
- Effective verbal and written communication skills
Preferred Skills:
- Experience working with telehealth platforms and remote monitoring tools
- Ability to collaborate with cross-functional teams including clinical and IT
- Strong problem-solving and coordination skills
