Job Summary

We are urgently hiring compassionate dependable Home Health Aides (HHA) to join our Stratford Connecticut home care team. This role provides the opportunity to deliver one-to-one personal care in patients homes support independence and build meaningful relationships. Both Full-Time and Per Diem schedules are available.

Responsibilities

  • Provide personal care services including bathing grooming dressing toileting and mobility assistance

  • Assist with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) such as meal preparation light housekeeping and errands

  • Support medication reminders as directed by licensed nursing staff (not administering medications unless certified and permitted)

  • Monitor and report changes in patient condition behavior or environment to supervisory clinical staff

  • Follow individualized care plans and participate in patient/caregiver teaching when requested

  • Document care observations and any incidents accurately and on time in the electronic record

  • Maintain patient dignity privacy and safety at all times and uphold professional boundaries

  • Travel to clients homes throughout the assigned service area; punctuality and reliable attendance required

Qualifications

  • High school diploma or equivalent preferred

  • Current Home Health Aide certification or state equivalent required

  • Preferred: 1 year of experience in home care long-term care or related direct care setting

  • Compassionate patient-centered approach with strong interpersonal skills

  • Basic clinical and safety knowledge including infection control and safe transfer techniques

  • Reliable transportation valid drivers license and proof of auto insurance as required for travel to patient homes

  • Ability to pass background checks and required health screenings

  • Excellent communication time management and documentation skills

  • Bilingual (Spanish) is a plus

Schedule & Compensation

  • Full-Time and Per Diem opportunities available
