Job Summary
Join our well-established Home Health team as an LPN!
Available: Full-time/ Part-time/ Per-Diem
Location: New Haven County CT and surrounding areas
This role offers the autonomy of field work the support of an experienced clinical staff and the opportunity to build meaningful one-on-one patient relationships.
Schedule:
MondayFriday daytime hours for strong worklife balance
Extra pay for rotating weekend/holiday coverage
Flexible scheduling options available
Qualifications:
Active Connecticut LPN license (required)
Minimum 6 months of clinical experience (hospital LTC SNF rehab med-surg or similar)
Valid drivers license reliable vehicle and can travel to visit clients
Strong assessment communication and documentation skills
Spanish speaking is a plus!
Home health experience is a plus!
Responsibilities:
Provide skilled nursing visits in the patients home
Document vitals as well as care during visits and report changes in condition
Implement and update individualized care plans under RN supervision
Deliver patient and caregiver teaching
Assist with medication management and wound care
Communicate effectively with physicians RNs therapists and the care team
Maintain professional boundaries and promote patient safety at all times
