Job Summary:
The Registered Nurse is responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. The role involves conducting comprehensive patient assessments administering medications and developing treatment plans while maintaining strong communication and collaboration with patients providers and the interdisciplinary care team.

Location:Honolulu Hawaii United States
Responsibilities:
- Provide direct skilled nursing care case management and hospice care to patients.
- Perform daily clinical assessments and monitor patient conditions.
- Administer medications safely and manage individualized treatment plans.
- Collaborate effectively with providers patients and interdisciplinary team members.
- Complete clinical documentation and administrative duties related to case management.
- Travel to various patient locations to deliver field-based care.

Required Skills & Certifications:
- Active Registered Nurse license in the state of practice.
- Minimum of two years of experience in hospital home health or hospice care.
- Bachelors degree in Nursing or a related field.
- Valid drivers license active car insurance and a personal automobile for field work.
- Demonstrated proficiency in clinical assessment medication administration and treatment planning.
- Excellent communication interpersonal and interdisciplinary collaboration skills.
- Active Registered Nurse license in the state of practice.

Scheduling:
- 40 hours per week.
- Rotating shifts.
