Role Summary

As the Pharmacy Clinical Coordinator at Howard University Hospital you will lead the development implementation and evaluation of clinical pharmacy services that support safe effective and evidence-based medication therapy across the hospital. Reporting to the Director of Pharmacy you will serve as a clinical leader and medication safety champion partnering with interdisciplinary care teams advancing formulary and medication-use initiatives and ensuring compliance with regulatory and quality standards.

What Youll Do

Medication Safety Leadership

  • Develop lead and maintain a robust organizational medication safety program aligned with ASHP guidelines and High Reliability Organization (HRO) principles
  • Serve as the primary expert for safe medication use and systems-based error reduction
  • Chair or co-chair the Medication Safety Committee; manage agendas workflows and follow-up actions
  • Develop medication safety alerts newsletters and communications for staff; promote a Just Culture of transparency reporting and learning from events

Policy Compliance & Regulatory

  • Develop revise and implement medication-use policies procedures and standard operating procedures (SOPs)
  • Ensure compliance with CMS Joint Commission DC Board of Pharmacy DSCSA and internal standards
  • Monitor national medication safety alerts (FDA ISMP ASHP manufacturers) and implement organization-wide safety actions

Pharmacy Technology & Informatics

  • Partner with Pharmacy Informatics IT and Nursing Informatics to optimize and ensure safe use of medication-related technology including EHR/CPOE order sets smart infusion pumps and drug libraries automated dispensing cabinets barcoded medication administration (BCMA) and clinical decision support (CDS)
  • Lead safety assessments for technology upgrades drug library updates new equipment and automation changes

Formulary & P&T Support

  • Provide oversight and clinical expertise to support formulary management processes
  • Participate in the Pharmacy & Therapeutics (P&T) Committee; prepare drug monographs therapeutic class reviews and medication-use evaluations (MUEs)
  • Work closely with P&T to evaluate medication error data identify formulary-related safety issues and recommend changes
  • Partner with Pharmacy Operations to ensure safe processes for ordering preparation compounding dispensing and distribution

Data Analytics & Performance Improvement

  • Collect trend and analyze medication-use safety data including event reports trigger tools audits and technology logs
  • Develop dashboards and reports for senior leadership and the Medication Safety Committee

Education & Preceptorship

  • Provide medication safety education to pharmacists nurses physicians and other staff
  • Serve as a preceptor to pharmacy students and residents; support their development as future practitioners

Required Qualifications

  • Doctor of Pharmacy (PharmD) required
  • Completion of a PGY-1 Pharmacy Residency required OR equivalent clinical hospital experience
  • Minimum of three (3) to five (5) years of direct clinical pharmacy experience in a hospital or health-system setting
  • Demonstrated experience participating in interdisciplinary rounds and providing direct patient care
  • At least one (1) to two (2) years of experience providing clinical guidance project leadership or informal supervision of pharmacists students or residents
  • Active registration as a pharmacist in the District of Columbia or eligibility for registration through reciprocity
  • Strong written oral and presentation skills; proven ability to build consensus among diverse stakeholders with competing priorities
  • Proficiency in Microsoft Office Suite (Excel Access)

Preferred Qualifications

  • Completion of a PGY-2 Residency in a clinical specialty (e.g. Internal Medicine Critical Care Infectious Diseases Cardiology)
  • Board Certification (BCPS BCCCP BCIDP BCACP or related)
  • Experience leading clinical programs developing protocols or supporting P&T activities
  • Experience precepting pharmacy students or residents
  • Experience in quality improvement clinical guideline development or medication-use evaluation
  • Familiarity with national medication safety and practice standards (ASHP ISMP Joint Commission)
  • Experience with Cerner
