What You Will Do

Manage your own caseload from admission through discharge
Complete Start of Care OASIS and routine visits
Develop and adjust individualized care plans
Educate patients and families to improve outcomes
Partner with physicians and interdisciplinary teams
Document efficiently using HomeCare HomeBase

What You Need

Active RN license
1 year nursing experience Home Health preferred
Strong assessment and critical thinking skills
Ability to work independently in the field
Reliable transportation and willingness to travel within territory

Ability to frequently lift push pull and support up to 50 pound
HomeCare HomeBase experience preferred
