Our mission: Through the talents of an experienced and diverse workforce OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy safe living which positively impacts our community.

We are seeking an Information & Referral Specialist to provide information to our consumers in response to service and eligibility inquiries.

What youll do:

  • Assess the health and functional status of members
  • Develop community- based care plans and related service packages
  • Participate in the development of a members Individual Care Plan (ICP)
  • Arrange coordinate and monitor the provision of appropriate members
  • participate in the determination of a members need for institutional and community long term care services

What youll bring:

  • Your passion for helping others and desire to make a difference.
  • Bachelors degree in social work or related field highly preferred; current licensure if appropriate
  • Minimum of two years professional experience in the care of people 65 and older
  • Current Social Work license (LSW) or higher preferred
  • Working knowledge of social emotional and physical process of aging
  • Ability to obtain information of a personal nature through interviews
  • Ability to gain and hold the confidence of clients and their families
  • Draw appropriate conclusions and make recommendations
  • Knowledge and skill necessary to work successfully within an interdisciplinary team
  • Excellent written and verbal skills.
  • Excellent computer skills.
  • Proficient with Microsoft Office Suite or related software.
