MUST HAVE CPR CERTIFICATE NATIONAL FEDERATION CONCUSSION TRAINING COURSE CERTIFICATE A SIGNED ANTI-HAZING TRAINING AFFIDAVIT AND A CORI (CRIMINAL OFFENDER RECORD INFORMATION).
