Summary

Reporting to the Site Lead for Social Work at St. Pauls Hospital and collaborating with Social Work Site Leaders across sites/programs and/or the Research Education and Practice Coordinator:

  • supports Providence Health Care (PHC) inter-professional programs/teams as the adult protection educator and subject matter expert
  • provides clinical education consultation and support to ensure organizational compliance with the Adult Guardianship Act (AGA)
  • leads the development evaluation and continuous improvement of PHCs Adult Guardianship Protection Program
  • manages the planning revision and delivery of ReACT Adult Protection education modules for Social Work staff across PHC
  • oversees clinical and consultative services by providing leadership and guidance on adult protection cases policy development and implementation
  • ensures assessment and alignment of procedures related to adult abuse neglect and self-neglect in accordance with AGA and Designated Agency obligations
  • represents PHC at provincial and regional committees (e.g. AGAPAC Regional Adult Protection Committee HAAGCOP)
  • develops and maintains strategic partnerships with internal and external stakeholders including patients and families government representatives legal counsel insurers and community agencies (e.g. Vancouver Police BC Public Guardian and Trustee ReAct Adult Protection Program Community Living BC)

What You Bring

Education Training and Experience

  • Masters Degree in Social Work from an accredited university
  • post-graduate continuing education in working with vulnerable/incapable adults including completion of the ReAct Adult Protection Curriculum (Modules 110)
  • five (5) years of recent related experience working with adult abuse neglect and vulnerable adult populations including one (1) year of teaching preceptoring and/or coaching experience
  • qualified health care provider (as defined by AGS 2.1) with experience conducting incapability assessments under statutory property guardianship (AGA 2.1) and health care consent and care facility admissions legislation or an equivalent combination of education training and experience
  • current full registration with the British Columbia College of Social Workers

Your Day to Day

  • develops plans and evaluates the ongoing enhancement of the Adult Guardianship Protection Program across PHC integrating best practices in adult education and ensuring compliance with the Adult Guardianship Act (AGA)
  • oversees chart audits in collaboration with Risk Legal and Ethics to evaluate use of emergency assistance provisions analyzes trends and provides recommendations to improve practice and staff learning
  • leads the development of policies practice guidelines and clinical tools related to adult protection work
  • delivers ReAct Adult Protection curriculum education identifies learning needs and supports orientation and ongoing staff development using varied teaching strategies
  • plans and coordinates education sessions tracks attendance evaluates program effectiveness and promotes continuous learning opportunities
  • acts as a clinical resource and mentor to staff and interdisciplinary teams providing guidance on complex cases involving abuse neglect and incapability assessments
  • builds and maintains strategic partnerships with internal and external stakeholders advocating for vulnerable adults and supporting coordinated care and safety planning
  • represents PHC on provincial and regional committees contributing to legislative policy and practice developments related to adult guardianship
  • participates in inter-professional care planning for patients with complex adult guardianship needs supporting assessment care planning and discharge coordination
  • contributes to quality improvement research and program evaluation initiatives including data tracking reporting and identifying opportunities to enhance service delivery
