Two exciting new positions have arisen at City of Liverpool College to co-ordinate the process of reviewing and maintaining Education Health and Care Plans (EHCP). This will include being the lead named person from the College with regards to receiving and requesting Education Health and Care Plans from local authorities and to monitor and review annually. The post holder will assist the Head of Learning Support and their deputy in arranging transition for learners with an EHCP. They will be responsible for identifying students who require High Needs Funding and liaising with the High Needs Funding Officer for applications for funding as well as working alongside our Learning Support managers to identify in-class support needs.

Key Responsibilities
- To be the Colleges named person to receive copies of Education and Health Care Plans from local authorities and to respond to consultations in a timely fashion in accordance with the SEND Code of Practice (2015)
- To assist the Head of Learning Support and their deputy in ensuring that ALS service standards are consistently upheld with regards to EHCPs and High Needs Funding
- To assist in the implementation and review of the LS delivery plan with regards to EHCPs
- Manage records of all EHCP dates of receipt response date and dates of review meetings in line with SEND Code of Practice (2015)
- Coordinate the collection collation completion and distribution of all relevant documents and data
- To review Education and Health Care plans with the relevant local authorities annually. Arranging meetings with parents tutors and local authority representatives when necessary
- Complete all relevant paperwork following the review meeting and submit to relevant Local Authority
- Work with professionals to ensure their knowledge expertise and skills inform the EHCP and annual reviews
- Liaise sensitively and effectively with young person/family/ external agencies/providers
- Liaising with Learning Support staff Head/Deputy Heads of School Progress Leaders and Tutors with regards to students with a EHCP and where necessary refer students to the appropriate external agencies for further support
- Participate and contribute to meetings regarding students with a EHCP with appropriate outside agencies under direction of the Head of Learning Support e.g. Ed Psych Speech and Language service ASC service EWO; LEA etc. and to facilitate opportunities for external agencies to work with students and staff when appropriate
- Ensure the EHCP clearly details the needs of the child/young person and has clear actions and outcomes relevant to the setting
- Link with Learning Support managers to share advice to Learning Support Practitioners (LSPs) of the needs of students with a EHCP and of suitable methods and strategies to support these needs
- Ensure high levels of confidentiality
- To assist in coordinating assessments of support needs for students with Education Health and Care Plans.
- Identifying students with a EHCP who might require higher levels of support and be potentially eligible for High Needs Funding
- To assists with the collection and collation of information for High Needs Funding Claims for students with EHCP
- Raising standards of EHCP student inclusion attainment and achievement by monitoring and supporting student progress
- To work closely with the Deputy Head of Learning Support and the Learning Support managers in identifying planning and delivery of enhanced transition for students with EHCP where applicable
- Share responsibility with Learning Support managers for attending meetings and communicating outcomes for EHCP students and monitoring progress in year
- To liaise with Learning Support managers with arrangements for initial assessments/interviews
- Keep and maintain accurate records of students with a EHCP
- Use ICT packages to support efficient and effective practice
- To assist in ensuring the accuracy and quality of student records
- Take every opportunity to positively engage with parents and carers forming strong links and communication regarding all SEND issues related to their children
- To be an effective team member assist in the running of team meetings and participate in continuing professional development.
- To take proactive responsibility for Health & Safety to ensure that a safe working environment and safe working practices are maintained at all times.
- To support cross college activities as required e.g. the key enrolment and invigilation periods.
- To comply with the General Data Protection Regulations (GDPR).
- To safeguard and promoting the welfare of young learners and vulnerable adults the post-holder comes into contact with.
- To demonstrate a positive commitment to the implementation of the Colleges Equality and Diversity Policy and to the maintenance of a culture of continuous quality improvement and innovation
- To undertake such other duties commensurate with the grade of the post as may reasonably be required at the initial place of work or at other locations in the College catchment area
Skills Knowledge and Expertise
Essential:
- Minimum Grade C (4 or above) in English and Maths or equivalent
- Relevant specialist qualification or experience in the SEN field
- Knowledge of SEND reforms Code of Practice and EHCP process
- Experience of working effectively with people with learning difficulties/disabilities in an education support capacity
- Experience and knowledge of conducting student centred annual and emergency reviews
- To have knowledge and commitment to inclusive learning
- Demonstrate commitment to effective working relationships with a range of different agencies
- Ability to implement maintain and manage accurate information retention systems both manual and computerised
- Experience of maintaining records in such areas as additional learning support funding claims and the monitoring of student progress
- Good organisational and IT skills
- Good presentation and written communication skills
- Ability to work under pressure organise and prioritise work and meet deadlines
- Ability to work as part of a team and on own initiative
Desirable:
- Experience of consultations and reviews in relation to EHCPs
- Knowledge and working experience of ProSolution and ProMonitor (College MIS and ILP systems)
