The team works collaboratively with hospitalists oncology critical care case management and community hospice agencies to ensure seamless high-quality care delivery.
Position Summary
The organization is seeking an experienced Hospice & Palliative Care Physician who can also provide Hospitalist coverage to support its expanding inpatient and palliative care services in Shelton Connecticut.
This is a full-time permanent opportunity focused on managing hospitalized patients requiring palliative consultations while also participating in general inpatient medicine coverage. Essential Responsibilities
Provide inpatient palliative care consultations including symptom management and goals-of-care discussions
Lead and participate in family meetings and interdisciplinary care planning
Deliver evidence-based hospitalist care during assigned inpatient shifts
Facilitate transitions to hospice care and coordinate with external hospice agencies
Collaborate with nursing case management social work and specialty teams
Maintain accurate timely documentation within the EMR
Participate in quality improvement initiatives and clinical reviews
Support a multidisciplinary patient-centered care environment
Qualifications
Education: MD or DO from an accredited medical school
Training: Residency in Internal Medicine or Family Medicine
Certification: Board Certified in Internal Medicine or Family Medicine required; Hospice & Palliative Medicine training or significant experience preferred
Licensure: Eligible for Connecticut medical licensure
Experience: Prior hospitalist or inpatient experience strongly preferred (not open to new graduates)
Skills: Strong communication skills empathetic patient care approach and ability to work in a team-based setting
