Role Responsibilities

• Lead and oversee daily rehabilitation operations ensuring high-quality patient-centered therapy services

• Provide direct patient care as needed while maintaining program oversight

• Collaborate with the interdisciplinary team to develop and implement individualized treatment plans

• Mentor coach and develop therapy staff to promote professional growth and engagement

• Oversee documentation scheduling productivity quality assurance and regulatory compliance

• Participate in Medicare meetings and coordinate care with residents families and facility leadership

• Drive evidence-based practice innovation and continuous program improvement

Qualifications

• Licensed Physical Therapist (PT) in the state of Wisconsin

• Prior leadership or supervisory experience preferred

• Experience in skilled nursing or senior living settings required

• Strong organizational communication and problem-solving skills

• Passion for working with older adults and leading high-performing therapy teams
