P-Physical Therapist

Position: Physical Therapist (P-Physical Therapist)

Salary Range: $44-47 per hour

Hours: 35 hours per week Monday-Friday with weekend rotation

Facility: Skilled Nursing Facility

Overview:

We are seeking a highly skilled and compassionate Physical Therapist (P-Physical Therapist) to join our team at a skilled nursing facility. Our facility offers post-hospital short-term rehabilitation services as well as long-term and respite care for our patients. As a Physical Therapist you will play a crucial role in helping our patients regain their strength mobility and independence.

Key Responsibilities:

- Conduct initial evaluations and develop individualized treatment plans for patients based on their specific needs and goals

- Implement treatment plans using various techniques such as therapeutic exercises manual therapy and modalities

- Monitor and track patient progress adjusting treatment plans as needed

- Collaborate with other members of the healthcare team to provide comprehensive care for patients

- Educate patients and their families on exercises and techniques to continue their progress at home

- Maintain accurate and up-to-date patient records and documentation

- Stay current on industry developments and advancements in physical therapy techniques and technology

- Adhere to all ethical and legal standards in patient care and documentation

Qualifications:

- Valid state license to practice as a Physical Therapist

- Minimum of 2 years of experience in a skilled nursing facility setting preferred

- Strong knowledge of physical therapy principles theories and techniques

- Excellent communication and interpersonal skills

- Ability to work independently as well as part of a multidisciplinary team

- Compassionate and patient-focused approach to care

- Proficient in using electronic medical records and other healthcare technology

Salary and Benefits:
