Southwestern Behavioral Healthcare is seeking a full-time Addiction Services Case Manager to join our team. This role supports adults with chronic substance use histories who benefit from structured supportive care. The position focuses on coordinating transitional residential services providing case management and skills training and supporting clients throughout their recovery journey.

WHAT IS THIS POSITION RESPONSIBLE FOR
Evaluate and screen applicants appropriateness for admission to the program.
Coordinate admissions to the service as well as discharges from the program
Monitor living environment involvement in community living and self-help groups employment and payment of fees.
Maintain adequate inventory of food and household supplies.
Monitor utilization of services and adhere to quality care standards.
Monitor residents compliance with transitional residential services guidelines.
Coordinate reentry into the community.
Provide skills training and care coordination to Transitional Residential patients.
Develop appropriate treatment plans and goals and monitor patients progress toward treatment goals.
Coordinate patient care with other members of the treatment team.
Maintain contact with referral sources and other providers as necessary.
Maintain appropriate clinical documentation.
Provide care coordination to clients in other addiction programming (residential and outpatient) as needed.
Collaborate with nursing staff and collaborating physicians to ensure high quality of medical care for clients.
Develop and sustain relationships with other providers in the community to provide for on-going client needs.
Qualifications
MINIMUM QUALIFICATIONS:
Bachelors degree in Social Work Psychology Sociology Criminal Justice or other relateddegree required.
Experience working with adults with a substance use disorder is preferable.
Possess the ability to assess emergency situations and react in an appropriate andtherapeutic manner.
Must have valid drivers license and qualify for Companys vehicle liability insurance.
Candidates must pass required background checks including county/state checks CPS check sex offender registry check and drug screen.
