Job Summary

Job Title: Case Manager RN
Location: Sanford FL
Employment Type: Permanent

Compensation:$36.96/hour - $44.00/hour (Varies by experience)
Work Schedule
  • Shift: Days (Rotating Weekends)
  • Schedule: MondayFriday with every other Saturday (may move to every 3rd Saturday once fully staffed)
  • Shift Timings: 8:00 AM 7:30 PM range (varies by schedule)
Position Overview

The Case Manager RN is responsible for coordinating and managing patient care throughout the hospitalization process to ensure safe effective and timely transitions across the continuum of care. This role acts as a liaison between patients families physicians nursing staff and community resources supporting discharge planning care coordination and service utilization.

The position plays a critical role in improving patient outcomes ensuring regulatory compliance and enhancing continuity of care from admission through discharge across all inpatient units.

Key Responsibilities
  • Coordinate patient care throughout the hospitalization lifecycle
  • Conduct psychosocial assessments and discharge planning
  • Develop individualized care and transition plans
  • Educate patients and families on care plans benefits and expectations
  • Collaborate with physicians nursing teams and hospital leadership
  • Facilitate safe and timely patient discharges
  • Ensure continuity of care across post-acute services
Required Qualifications

Must-Have:

  • Active RN or APRN license
  • Associate Degree in Nursing or RN Diploma
  • Minimum 2 years of recent acute-care Case Management RN experience
  • Experience must be within the last year
  • Strong background in discharge planning and care coordination
