Position Overview
A reputable healthcare facility is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join its long-term care team. This role focuses on delivering high-quality patient care supervising support staff and ensuring compliance with healthcare regulations while maintaining a safe and supportive environment for residents.
Key Responsibilities
Provide direct nursing care based on individualized care plans and physician orders
Supervise and support nursing assistants and care staff in daily operations
Administer medications and treatments safely ensuring proper patient identification
Monitor patient conditions and maintain accurate clinical documentation
Transcribe and implement physician orders into medical records and care plans
Assist physicians during rounds and communicate updates effectively
Manage admissions transfers and discharges
Perform clinical procedures such as:
Wound care
Catheterization
Tube feeding
Suctioning
Specimen collection
Record and monitor vital signs (temperature pulse respiration blood pressure)
Maintain inventory of medications and medical supplies
Educate patients and families on care plans and health management
Participate in audits/surveys and ensure regulatory compliance
Maintain confidentiality in accordance with HIPAA standards
Support onboarding and orientation of new residents
Collaborate with interdisciplinary teams to ensure quality outcomes
Qualifications
Graduate of an accredited LPN/nursing program
Active unrestricted LPN license (Massachusetts)
Current BLS/CPR/AED certification
Strong English communication skills (written and verbal)
Knowledge of long-term care practices and regulations
Ability to work independently and supervise staff effectively
