Physical Therapist Responsibilities
Perform initial and ongoing evaluations of residents physical abilities mobility strength balance and functional status
Identify impairments and barriers to safe and independent mobility
Develop and implement individualized goal-driven physical therapy treatment plans
Address gait balance endurance and functional mobility needs
Support recovery following injury surgery illness or neurological conditions
Treat residents with chronic conditions such as arthritis Parkinsons disease osteoporosis or post-stroke deficits
Improve performance in functional activities including transfers walking stair negotiation and fall recovery
Provide therapeutic exercise neuromuscular re-education and manual therapy as appropriate
Recommend fit and educate residents on assistive devices and durable medical equipment
Perform fall risk assessments and implement fall prevention strategies
Educate residents families and staff on safe mobility techniques and home safety
Collaborate with interdisciplinary teams including OT SLP nursing and care staff
Participate in care planning meetings to support holistic resident outcomes
Maintain accurate timely documentation and ensure regulatory compliance

Qualifications
- Graduate of an accredited school with a minimum of a bachelors degree in Physical Therapy.
- Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.
