Physical Therapist Responsibilities

• Perform initial and ongoing evaluations of residents physical abilities mobility strength balance and functional status

• Identify impairments and barriers to safe and independent mobility

• Develop and implement individualized goal-driven physical therapy treatment plans

• Address gait balance endurance and functional mobility needs

• Support recovery following injury surgery illness or neurological conditions

• Treat residents with chronic conditions such as arthritis Parkinsons disease osteoporosis or post-stroke deficits

• Improve performance in functional activities including transfers walking stair negotiation and fall recovery

• Provide therapeutic exercise neuromuscular re-education and manual therapy as appropriate

• Recommend fit and educate residents on assistive devices and durable medical equipment

• Perform fall risk assessments and implement fall prevention strategies

• Educate residents families and staff on safe mobility techniques and home safety

• Collaborate with interdisciplinary teams including OT SLP nursing and care staff

• Participate in care planning meetings to support holistic resident outcomes

• Maintain accurate timely documentation and ensure regulatory compliance
