Job Summary
Corewell Healths Southeast service area is seeking an Advanced Practice Provider (APP) to join our dynamic team in a Visiting Site Specialist role. In this position you will provide care across the generational spectrum by supporting busy practices where provider coverage is needed. When not assigned to a visiting site your home base will be West Pavilion Primary Care Medicine.
2 Years of experience is REQUIRED. Family Nurse Practitioners or Physician Assistants are required as we care for patients of all ages.
We are seeking an individual who:
- Has a flexible working style as you will be working with multiple different teams
- Enjoys a flexible working location travel could include several different sites per week within the below coverage area. Efforts will be made to limit 2-3 sites per week
- Has a positive attitude and collaborative approach as you will be working in multiple different offices
- Wants a Monday through Friday 8:00 4:15 pm work schedule with an hour lunch
All practices have a mix of physicians and Advanced Practice Providers. Team-based care is supplemented with Behavioral Health Pharmacists Social Workers and experienced medical assistants working one-to-one with the providers. Ancillary services such as laboratory x-ray PT/OT and specialty care are available at most sites.

Other Sites include: Lake Drive 426 Michigan FM/IM/Walk-In Clinic IM and FM Residency Community Medicine Clinic ICC Beltline Coopersville Campustowne Gaslight Kentwood IM & FM South Pavilion 68th Street Pediatrics Byron Center Rivertown West Pavilion Georgetown Zeeland Holland FM/IM/Peds/Walk-In Clinic Grand Haven Caledonia.

Qualifications:
Nurse Practitioner:
- Required Masters Degree Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a masters post-masters or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
- 3 years of relevant experience current relevant clinical experience
- Previous experience functioning in a collaborative role as a Nurse Practitioner
- LIC-Nurse Practitioner (NP) - State of Michigan
- CRT-Basic Life Support (BLS)
- CRT-Neonatal Resuscitation Program (NRP)
- CRT-Pediatric Adv Life Support (PALS)
Physician Assistant:
- Required Masters Degree Graduate of an accredited Physician Assistant educational program
- LIC-Physician Assistant - State of Michigan
- CRT-Physician Asst Certified (PA-C) - NCCPA National Commission on Certification of Physician Assistants
- CRT-Basic Life Support (BLS)
- CRT-Pediatric Adv Life Support (PALS)
- CRT-Neonatal Resuscitation Program (NRP)
