Job Title: Medication Aide Home Care
Position Summary
The Medication Aide assists clients in their homes with the administration of prescribed medications in accordance with state regulations agency policies and the clients plan of care. The Medication Aide works under the supervision of a licensed nurse or other authorized healthcare professional to promote client safety independence and wellbeing. You must have a 10 hour course in adult home care medication aide not a certified medication technician for this position.
Essential Duties and Responsibilities
- Assist clients with selfadministration of medications as permitted by state law
- Administer medications within approved scope of practice and certification
- Follow physician orders medication administration records (MARs) and care plans
- Observe document and report medication effects side effects and changes in client condition
- Ensure medications are taken at the correct time dose and route
- Maintain accurate and timely documentation of medication assistance or administration
- Provide reminders and education to clients regarding medication purpose and schedule (as appropriate)
- Safely store handle and account for medications according to agency policy
- Report medication errors or concerns immediately to supervising staff
- Follow infection control safety and confidentiality standards
- Assist with basic activities of daily living (ADLs) as outlined in the care plan (if applicable)
Qualifications
Education and Certification
- High school diploma or equivalent required
- Completion of a stateapproved Medication Aide training program
- Current Medication Aide certification (as required by state regulations)
- CPR/First Aid certification preferred or required
Experience
- Previous experience in home care assisted living or healthcare setting preferred
- Experience working with elderly or disabled clients a plus
Knowledge Skills and Abilities
- Knowledge of basic medication administration principles
- Understanding of infection control and safety practices
- Ability to follow written and verbal instructions accurately
- Strong attention to detail and organizational skills
- Excellent communication and observation skills
- Compassionate respectful and clientcentered approach
- Ability to maintain confidentiality and professional boundaries
- Basic documentation and electronic recordkeeping skills
- Reliable transportation and ability to travel to client homes
Working Conditions
- Work performed in clients private homes
- May require evenings weekends or holidays
- Ability to stand bend lift and assist clients as needed
