Navitas Healthcare LLC is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a clinic setting. The LPN will provide patient-centered care support clinical assessments and collaborate with the healthcare team to deliver high-quality outpatient services.
Key Responsibilities
Perform and document patient assessments within LPN scope of practice
Assist with patient health histories and admission processes
Identify patient care needs and support care planning with the clinical team
Implement and monitor care plans under the direction of licensed practitioners
Observe report and respond to changes in patient condition
Administer medications treatments and nursing interventions safely and accurately
Maintain timely and accurate clinical documentation
Support discharge planning and patient education initiatives
Communicate effectively with patients families and interdisciplinary teams
Ensure proper task delegation while maintaining patient safety standards
Follow all facility policies procedures and clinical guidelines
Required Qualifications
Active Minnesota LPN License (required at submission)
Minimum 2 years of LPN experience preferably in clinic or outpatient settings
Experience with EPIC EMR preferred
Strong knowledge of clinical procedures and patient care practices
Excellent communication and interpersonal skills
Ability to work in a fast-paced team-oriented environment
Strong organizational skills and attention to detail
