Job Overview
Navitas Healthcare LLC is seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a clinic setting. The LPN will provide patient-centered care support clinical assessments and collaborate with the healthcare team to deliver high-quality outpatient services.
Key Responsibilities
- Perform and document patient assessments within LPN scope of practice
- Assist with patient health histories and admission processes
- Identify patient care needs and support care planning with the clinical team
- Implement and monitor care plans under the direction of licensed practitioners
- Observe report and respond to changes in patient condition
- Administer medications treatments and nursing interventions safely and accurately
- Maintain timely and accurate clinical documentation
- Support discharge planning and patient education initiatives
- Communicate effectively with patients families and interdisciplinary teams
- Ensure proper task delegation while maintaining patient safety standards
- Follow all facility policies procedures and clinical guidelines
Required Qualifications
- Active Minnesota LPN License (required at submission)
- Minimum 2 years of LPN experience preferably in clinic or outpatient settings
- Experience with EPIC EMR preferred
- Strong knowledge of clinical procedures and patient care practices
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced team-oriented environment
- Strong organizational skills and attention to detail
