Job Summary

The Certified Peer Specialist/Certified Recovery Specialist supports CBH Clinical Department in the proper procurement of behavioral health services of CBH members. The Certified Peer Specialist/Certified Recovery Specialist will assist members in connecting to physical and behavioral health services and community resources.

The Certified Peer Specialist/Certified Recovery Specialist will remain connected and conduct follow up to members transitioning out of ASAM bed-based levels of care.


Essential Functions:

  • Build a rapport with the members identified for CPS/CRS services by the ASAM Team.
  • Meet with members to obtain feedback about service engagement and to provide encouragement emotional support and positive feedback to reinforce positive health actions/behaviors.
  • Coordinate and oversees a recovery plan developed with member.
  • Conduct follow up with member in community to review members understanding of continued recovery efforts medication and follow-up needs.
  • Conduct telephonic follow-up and in-person meetings with member at either provider locations or in the community.
  • Assist member to ensure that tasks are performed such as attending medical or behavioral health appointments obtaining prescription medication attending support groups etc.
  • Monitor/identify potential barriers for recovery and community tenure; connect members to resources to address identified barriers and collaborate with the members treatment team to resolve barriers
  • Participate in routine individual provider meetings and network meetings to address member needs advocate for the voice of the members and promote resource sharing.
  • Collaborate with Clinical Care Managers to inform the care management process and ensure continuity of care.
  • Tracks and submits necessary report/data to provider on members progress.
  • Identify and maintain a library of resources that support the ASAM providers in addressing the needs of members.
  • Maintain clinical documentation in CBH Clinical Information System
  • Adhere to department productivity for CPS/CRS staff.

Position Requirements:

  • Active Certified Peer Specialist/Certified Recovery Specialist (CPS/CRS) certification
  • Relevant Work Experience: Possesses at least 2 years of direct service experience as a CPS/CRS.
  • PA Child Abuse Clearance required
  • Ability to travel throughout Philadelphia and the surrounding counties proficient in navigation of public transportation system

Skills:

  • Familiar with services for adults and children in the managed care system
  • Excellent interpersonal and collaboration skills
  • Excellent verbal and written communication skills
  • Proficiency with Microsoft Office and able to type 20 words per minute
  • Strong organizational skills with the ability to multi-task in a high-volume environment
  • Ability to work independently and as part of a team
  • Compliant with HIPAA regulations
  • Experience as a WRAP (Wellness Recovery Action Plan) Facilitator Preferred

Work Conditions (including travel overtime required physical requirements and occupational exposure):

  • Must be able to travel within Philadelphia and surrounding counties.
  • Regularly requires walking standing stooping bending sitting reaching pushing pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
  • Ability to utilize computers and office equipment to complete daily work responsibilities.
  • Requires continuous normal hearing and vision.
  • Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
  • Must be able to adapt to continuous changes/demands of the job.

Philadelphia Residency Requirement:

  • The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
