Job Overview:

We are seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality patient care in a long-term care setting. This role is ideal for a compassionate and detail-oriented nurse committed to delivering patient-centered care and improving patient outcomes.

Key Responsibilities:

• Plan implement and document patient-centered nursing care
• Perform patient assessments and develop individualized care plans
• Deliver nursing interventions based on best practices and clinical guidelines
• Monitor patient conditions and provide therapeutic care
• Maintain accurate and timely clinical documentation
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Support continuous improvement and evidence-based practices
• Ensure compliance with healthcare regulations and safety standards
• Perform additional duties as assigned

Requirements:

Experience:

• Minimum 2 years of recent LPN experience in long-term care rehabilitation or similar clinical setting

Licenses/Certifications:

• Active New York LPN license
• BLS certification (American Heart Association required)

Work Environment:

Fast-paced long-term care environment requiring strong clinical skills and teamwork
